Your medical health and contact details
As treatment may have to be altered to suit your needs your medical health information is vitally important to us. We will ask you to complete a medical questionnaire every 2 years. Please let us know if you feel we need to be informed about changes to your health.
Please download and print a copy of our change of details form to let us know if your contact details change. You may also use our online booking form to notify us of a change of address.
